PATIENT SERVICE CENTER:

Effective October 6, 2014, upon request by a patient (or the patient’s personal representative), laboratory may provide patients, their personal representatives, and those persons specified under 45 C.F.R.164.524 (c) (3) (ii) persons designated by the patient, as applicable, with access to completed test reports that, using the laboratory’s authentication process, can be identified as belonging to that patient.

Clin-Path Diagnostics will disclose completed test results to only:

  • An authorized person responsible for using the test results in a treatment context. An authorized person is an individual under state law who is authorized to order or receive test results, or both.
  • The clinician’s office that initiated the test(s).
  • The patient or the patient’s representative, upon request. The patient, and personal representative as applicable, must meet the authentication requirements as outlined in the policy.

Clin-Path Diagnostics may require individuals to make all requests for information (including those initiated by phone call) in writing. Please allow up to 30 days for this request to be processed once it is received by CPD. The required form can be found on the link below and faxed to 480-478-8091 or mailed to:
